PROFILE.

members of current opening(s) in your company.
Please fill out the following information and submit it. Alternatively,
make a copy of the Fact Sheet and fax or mail it to:

If Profile numbers are entered, all information entered on the form is sent to the JSFG office to be send to the
owner of each Profile indicated.
If no Profile numbers are entered, all information entered is sent to the JSFG mailing list.
